1. SW /designee will inform residents, families and resident representatives of outdoor visiting policy/procedure as detailed below 

  2. Facility visitation will be conducted in outdoor area, weather permitting.  

  3. In inclement weather such as high heat and as facility space allows visitation will be inside, in a well-ventilated space with no more than 10 individuals who are socially distanced. 

  4. Visits will be scheduled in advance by the Social Worker/Activities staff. (Refer to the attached schedule.) 

  5. The daily Visitation schedule will be reviewed at the Morning QI Meeting to ensure that all residents are aware of and ready for their visitation time. 

  6. The facility will assign staff to assist with the transportation of residents, monitoring of visitation, and cleaning/disinfecting areas used for visitation after each visit. 

  7. The facility will post signage regarding facemask utilization and hand hygiene and uses applicable floor markings for social distancing. 

  8. The facility will screen all visitors for signs and symptoms of COVID-19 prior to resident access and visitation will be refused if the individual(s) exhibits any COVID-19 symptoms. This will include temperature checks and screening questions to assess potential exposure to COVID-19, international travel and to states designated under the Commissioner’s travel advisory. The facility must maintain screening question asked onsite in an electronic format and make it available upon NYS DOH  request. 

    •  A log will be kept for all visitors that includes: 

      1. First and last name of the visitor; 

      2. Physical (street) address of the visitor; 

      3. Daytime and evening telephone number 

      4. Date and time of visit; 

      5. Email address, if available; and 

      6. As per NYSDOH a notation indicating the individual cleared the screening (both temperature and questions) that does not include any individual temperatures or other individual specific information.  

      7. Verification of Negative COVID test result presented 

  9. Effective 9/24/2020, visitors are required to present a verified negative test result within the past seven days.

  10. Visitors and residents must wear a facemask or face covering (must cover both the nose and mouth when on the premises of the facility). Masks will be available for visitors as needed.  

  11. Visiting areas will have easily accessible alcohol-based hand rub, for residents, visitors, and staff.  

  12. No more than 10 percent of the residents shall have visitors at any one time and only two visitors will be allowed per resident at any one time.  

  13. Visitors under the age of 18 must be accompanied by an adult 18 years of age or older.

  14. Each visit will be limited to 30 minutes unless previously approved by facility for additional time. 

  15. Visitation will be prioritized based on most recent visit, availability of open time sessions, facility census and staff availability.  

  16. Residents will be transported to and from the visitation area by assigned staff. 

    • Residents will wear a face mask during transport to and from visitation and at all times during the visit.  

    • Resident will be assisted with hand hygiene prior to and after visitation session. 

  17. Current COVID-19 positive residents, residents with COVID-19 signs or symptoms, and residents in a 14-day quarantine or observation period are not eligible for visits.  

  18. Visitors will be provided with a fact sheet outlining visitor expectations including appropriate hand hygiene and face coverings. The fact sheet will be provided upon initial screening to all visitors.  

  19. The IDT Team will review the Visiting program and monitor for any needed adjustments and report to QA Committee as needed. 

  20. If any visitor fails to adhere to the protocol, he/she/they will be prohibited from visiting for the duration of the COVID-19 state declared public health emergency.
